[摘要]
目的:探讨进展期胃癌淋巴结(LN)转移的规律，指导进展期胃癌LN 廓清手术治疗。方法:回顾性
分析2009 － 09 ～ 2012 － 09 间在我院手术并有完整记录的158 例进展期胃癌病人的临床资料，分析其LN 转移情
况。结果:肿瘤直径＞ 5cm、3cm ＜ 肿瘤直径≤5cm 的LN 转移率分别为91． 4% (64 /70)、78． 7% (48 /61)，明显高
于肿瘤直径≤3cm 的29． 6%(8 /27)(P ＜ 0． 01)。各组LN 中以第3 组LN 转移率最高为51． 6%，其他依次为第1、
2、7、8、12、13 组LN，而第14、15 组LN 对于不同部位的胃癌转移机会很少。结论:肿瘤大小、浸润深度是影响进
展期胃癌LN 转移的因素，掌握进展期胃癌LN 转移的规律，对进展期胃癌LN 的廓清手术具有重要意义。

[Abstract]
Objective:To explore the law of LN metastasis in advanced gastric cancer，guiding LN
dissection for advanced gastric surgery． Methods:To analyze their LN metastasis，surgery and complete
records of 158 patients diagnosed as advanced gastric cancer from September 2009 to September 2012
in our hospital． Results:Tumor diameter ＞ 5cm，3cm ＜ tumor diameter ≤5cm LN metastasis rate was
91． 4%(64 /70)，78． 7%(48 /61)，significantly higher than the tumor diameter ≤3cm of 29． 6% (8 /
27)(P ＜ 0． 01)． The 3rd group LN in the groups LN metastasis rate up to 51． 6%，followed by the 1st，
2nd，7th，8，12，13 group LN，and the 14th，15 group LN for different parts of the gastric cancer metastasis
opportunity seldom． Conclusions:Tumor size，depth of invasion are factors influencing LN metastasis
of advanced gastric，to master advanced gastric cancer LN metastasis law，the LN dissection surgery
for advanced gastric cancer is of great significance．
