[摘要]
目的: 回顾性分析18F-FDG PET/CT 对非霍奇金淋巴瘤( non-Hodgkin’s lymphoma,NHL) 化疗效果及
预后评估的临床应用价值。方法: 收集2011-01~ 2016-01 在内蒙古医科大学附属医院经病理检查确诊的NHL
病人108 例,化疗中期( 3~ 4 个疗程后) 行18F-FDG PET/CT 显像,并随访6 ~ 60mo,分析PET 和同机CT 的结果,
对病灶行临床综合评估,应用无进展生存( progression free survival,PFS) 期及总体生存( Overall survival,OS) 期作
为随访指标,进行疗效评价。率的比较采用χ2 检验,病人的生存分析采用Kaplan-Meier 方法。结果: 中期疗效
评价中,与临床综合评估结果比较,阳性58 例,阴性50 例。18F-FDG PET 灵敏度、特异性及准确性分别为82.8%
( 48 /58) 、78.0%( 39 /50) 及80.6%( 87 /108) ,而CT 检查分别为53.4%( 31 /58) 、64%( 32 /50) 及58.3%( 63 /108) ,18
F-FDG PET 对NHL 诊断的灵敏度、特异性及准确性均高于CT 检查,差异均有统计学意义( P<0.05) 。PET 阳性
者及阴性者的中位PFS( progression-free survival) 期分别35mo 和45mo,差异有统计学意义( χ2 = 31.71,P= 0.00) 。
结论: 18F-FDG PET/CT 显像在非霍奇金淋巴瘤疗效评价及预后判断有重要临床价值,优于传统影像学检查。

[Abstract]
Objective: To investigate the application value of 18F-fluordeoxyglucose positron emission
computed tomography /computed tomography( 18F-FDG PET /CT) in monitoring response to chemotherapy
and prognosis of non-Hodgkin’s lymphoma( NHL) .Methods: We collected 108 patients with
non-Hodgkin’s lymphoma who have been Confirmed by pathological examination in the Affiliated Hospital
of Inner Mongolia Medical University from January 2011 to January 2016,and were monitored by
18F-FDG PET /CT before chemotherapy and after 3~4 courses of chemotherapy.We follow up patients
in 6~60months.We analysed 18F-FDG PET /CT and CT scan in same machine,we comprehensive
evaluate lesion.Predict progression free survival( PFS) and overall survival( OS) ,we statistical analysis
the results by chi-square test and Kaplan-Meier.Results: We evaluated the lesion response to chemotherapy
at metaphase with clinical comprehensive analysis, it showed positive in 58 cases and negative
in 50 cases.the sensitivity, specificity,accuracy of 18F-FDG PET in evaluation of response to treatment
in NHL were 82.8%( 48 /58) 、78.0%( 39 /50) and 80.6%( 87 /108) ,which were significantly higher
than those of CT scan in same machine, It was 53.4%( 31 /58) 、64%( 32 /50) and 58.3%( 63 /108) ,P<
0.05.The progression-free survival( PFS) were 35months and 45months in PET positive and negative
patients respectively, the difference was statistically significant,( χ2 = 31.71,P = 0.00) .Conclusion: 18F
-FDG PET /CT has significant value in monitoring response to therapy and predicting prognosis for patients
with NHL.It is superior to traditional imaging.
