[摘要]
目的: 探讨过敏性紫癜患儿外周血血管细胞黏附分子－1、内皮生长因子及免疫球蛋白的含量变化在
过敏性紫癜的发病机制中的作用。方法: 选取HSP 患儿28 例、HSPN 患儿22 例和30 例健康儿童为对照组，采用
ELISA 的方法进行检测各组VCAM－1 和VEGF 的含量，同时采用全自动生化仪检测各组患儿免疫球蛋白水平。
比较HSP 组、HSPN 组和正常对照组之间VCAM－1、VEGF 和免疫球蛋白的差异，并进行相关性分析。结果: 血浆
VCAM－1 和VEGF 含量HSPN 组＞HSP 组＞正常对照组，差异有统计学意义( P＜0．01) ; HSPN 组、HSP 组与正常对
照组IgA 水平比较，差异有统计学意义( P＜0．05) 。HSPN 组与HSP 组IgA 水平比较及各组间IgM 和IgG 水平比
较差异无统计学意义( P＞0．05) 。血浆中VCAM－1 和VEGF 水平均与IgA 水平呈正相关，VCAM－1 和VEGF 之间
也呈正相关( 均r＞0，P＜0．05) 。结论: VCAM－1 和VEGF 均参与了HSP 血管炎的发生过程，同时与肾脏损害的程
度密切相关。

[Abstract]
Objective: To explore the effect of the changes in the content of endothelial growth
factor and immunoglobulin in peripheral blood vascular cells of allergic purpura in the pathogenesis of
allergic purpura．Methods: 28 cases children with HSP， 22 cases of children with HSPN and 30 cases of
healthy children as control group，adopt the method of ELISA test groups VCAM－1 and the content of
VEGF，at the same time，using automatic biochemical analyzer test groups with immunoglobulin levels．
The differences of VCAM－1，VEGF and immunoglobulin between HSP group，HSPN group and normal
control group were compared and the correlation analysis was conducted． Ｒesults: There were
statistically significant differences between the plasma VCAM－1 and VEGF content HSPN grou P＞HSP
grou P＞，and the difference was statistically significant( P＜0．01) ．There was a statistically significant
difference between HSPN group and HSP group and the normal control group IgA( P＜0．05) ．There was
no statistically significant difference between HSPN group and HSP group IgA level and between the
IgM and IgG levels( P＞0．05) ．The levels of VCAM－1 and VEGF in plasma were positively correlated
with IgA level，and there was positive correlation between VCAM－ 1 and VEGF( all r ＞ 0，P＜ 0．05) ．
Conclusion: VCAM－1 and VEGF are both involved in HSP vasculitis，and are closely related to the
degree of renal damage．
