目的: 探讨医疗纠纷发生的原因,提出相应的防范对策和建议,以期提高医院的管理水平。方法: 采
用回顾性流行病学调查的方法,对北京市10 家综合性医院2016-01 ~ 2017-12 出现的医疗纠纷进行分析。结
果: 2016 年2713 例医疗纠纷中,有2531 例得到了及时有效的处理,处理率为93.3%; 2016 年2319 例医疗纠纷中,
2159 例得到了有效的处理,处理率为93.1%。医疗纠纷涉及的主要人员中男性居多,以医保病人、副高及以上医
师、内外科科室发生居多。在所有因素中,以医疗过失纠纷中的医方服务沟通不畅和非医疗过失纠纷中的医疗
并发症所导致的纠纷为主,2016 年分别29.4%和23.3%; 2017 年分别为28.4和26.2。其次为医方技术事故导致的
纠纷,2016 与2017 分别占16.8%和17.6%。2017 年非医疗纠纷发生率增加。医患双方协商为处理医疗纠纷的主
要方式,2016 年与2017 年分别占52.6%、55.8%。结论: 医疗过失纠纷是医疗纠纷的主要原因,其中医方服务沟
通不畅引发的医疗纠纷比率最高。

Objective: To explore the causes of medical disputes and to put forward corresponding
countermeasures and suggestions in order to improve the management level of the hospital.Methods: A
retrospective epidemiological survey was used to analyze the medical disputes in 10 comprehensive hospitals
in Beijing in January 2016 ~ 2017 December.Results: In 2016,2713 cases of medical disputes,
2531 cases were dealt with promptly and effectively, the treatment rate was 93.3%.In 2017, 2319 cases
of medical disputes, 2159 cases were effectively dealt with, the treatment rate was 93.1%.The majority
of the main personnel involved in medical disputes are the majority of the men, including medical insurance,
the subtropical high and above,and the Department of internal and external sciences.In all factors,
the medical complications of medical negligence disputes in the medical service of poor communication
and non-medical negligence disputes caused by the dispute, in 201629.4%and 201723.3%; 28.4
and 26.2respectively.Followed by the medical accident technical disputes,2016 and 2017 respectively
accounted for 16.8%and 17.6%.The incidence of non-medical disputes increased in 2017.The consultation
between doctors and patients was the main way to deal with medical disputes,which accounted
for 52.6% and 55.8% respectively in 2016 and 2017. Conclusion: The medical dispute is the main
reason of medical disputes, the medical service of poor communication caused by the highest rate of
medical disputes.
