[摘要] 目的 探讨损伤控制理论(DCO)对严重多发性骨关节创伤患者凝血功能及术后并发症发生率的影响。方法 选取2017年2月~2019年2月我院严重多发性骨关节创伤患者116例,按治疗方案不同分为试验组、对照组,各58例。对照组采取常规救治,试验组采取DCO救治。比较两组手术情况、并发症发生情况、病死率与术前术后3 d凝血功能指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)]水平,并分析两组不同损伤严重程度评分(ISS)患者病死情况。结果 (1)手术情况:试验组手术时间、住院时间短于对照组,手术成功率高于对照组(P<0.05);(2)并发症:试验组并发症发生率低于对照组(P<0.05);(3)病死情况:试验组病死率低于对照组(P<0.05);(4)凝血功能指标:术后3 d试验组PT、APTT水平低于对照组(P<0.05);(5)不同ISS评分患者病死情况:试验组ISS评分为31~50分患者病死率低于对照组(P<0.05)。结论 DCO应用于严重多发性骨关节创伤患者,有利于减少并发症,改善凝血功能,提高手术成功率,降低病死率

[Abstract] Objective To investigate the effect of injury control theory (DCO) on coagulation function and postoperative complication rate in patients with severe multiple bone and joint trauma. Methods A total of 116 patients with severe multiple bone and joint trauma in our hospital from February 2017 to February 2019 were enrolled. According to different treatment options, they were divided into experimental group and control group, with 58 cases in each group. The control group received routine treatment and the experimental group received DCO treatment. The operation status, complications, mortality and 3 days postoperative coagulation function indexes [prothrombin time (PT), activated partial thromboplastin time (APTT)] were compared between the two groups. Severity score (ISS) patient mortality. Results (1) Surgery: The operation time and hospitalization time of the experimental group were shorter than that of the control group, and the success rate of surgery was higher than that of the control group (P<0.05). (2) Complications: The incidence of complications in the experimental group was lower than that of the control group (P<0.05); (3) Case of death: The mortality rate of the test group was lower than that of the control group (P<0.05); (4) The index of coagulation function: The level of PT and APTT in the test group was lower than that of the control group (P<0.05). (5) Patients with different ISS scores: The mortality rate of the patients with ISS score of 31-50 was lower than that of the control group (P<0.05). Conclusion DCO is used in patients with severe multiple bone and joint trauma, which is beneficial to reduce complications, improve coagulation function, improve the success rate of surgery and reduce the mortality rate.
