[摘要] 目的 观察血清白蛋白(ALB)、C反应蛋白(CRP)及生长分化因子-15(GDF-15)在川崎病患儿机体中表达情况,并分析上述因子水平与川崎病合并冠脉损伤之间的相关性,旨在为川崎病患儿冠脉损伤的防治提供科学的参考依据。方法 整群抽样法,回顾性分析我院2017年6月至2019年6月期间收治的65例川崎病患儿的临床资料,将其中经冠状动脉超声检查明确冠脉损伤的35例患儿纳入冠脉损伤组,将其余未发生冠脉损伤的30例患儿纳入无冠状动脉损伤组,另选取同时期内体检健康的30例儿童纳入对照组。对比各组ALB、CRP、GDF-15表达水平,再根据相关冠脉扩张标准,分析不同冠脉扩张程度与ALB、CRP、GDF-15之间的相关性。结果 冠脉损伤组ALB水平<无冠脉损伤组<对照组,冠脉损伤组CRP、GDF-15水平>无冠脉损伤组>对照组,差异有统计学意义(P<0.05);相比冠脉扩张内径≥3mm、≥2.5mm,≥3.5mm患儿ALB水平较低,CRP、GDF-15水平均较高,差异有统计学意义(P<0.05);经双变量Pearson相关性分析检验证实,ALB与冠脉扩张程度呈负相关(r=-0.407,P<0.05),CRP、GDF-15与冠脉扩张程度呈正相关(r=0.672、0.225,P<0.05)。结论 CRP、GDF-15水平在川崎病患儿中呈高表达,ALB呈低表达,且ALB、CRP、GDF-15表达水平与川崎病患儿冠脉损伤程度之间密切相关,临床可通过检测川崎病患儿ALB、CRP、GDF-15表达水平,评估患儿是否发生冠脉损伤及其损伤程度,并对其及时采取干预性治疗,以提高患儿预后。

[Abstract] Objective To observe the expression of serum albumin, C-reactive protein (CRP) and growth differentiation factor-15 (GDF-15) in children with Kawasaki disease, to analyze the correlation between the levels of above factors and coronary artery lesions, and to provide scientific basis for prevention and treatment of children with Kawasaki disease. Methods The cluster sampling method was adopted. The clinical data of 65 children with Kawasaki disease who admitted in the hospital from June 2017 to June 2019 were retrospectively analyzed. 35 children with coronary artery lesions examined by coronary ultrasound were included in coronary artery lesion group, 30 children without coronary artery lesions were included in no-coronary artery lesion group, and 30 healthy children with physical examination during the same period were included in control group. The levels of ALB, CRP, GDF-15 in three groups were compared. According to the relevant coronary dilation criteria, the correlation between different degrees of coronary dilation and ALB, CRP, GDF-15 was analyzed. Results The level of ALB in coronary artery lesion group was lower than that in no-coronary artery lesion group and control group, and the levels of CRP and GDF-15 in coronary artery lesion group were higher than those in no-coronary artery lesion group and control group (P<0.05); The levels of ALB in children with coronary dilation diameter≥3mm, ≥2.5mm, ≥3.5mm were lower, and the levels of CRP and GDF-15 were higher (P<0.05); Bivariate Pearson correlation analysis confirmed that ALB had negative correlation with the degree of coronary dilation (r=-0.407, P<0.05), and CRP, GDF-15 had positive correlation with the degree of coronary dilation (r=0.672, 0.225, P<0.05). Conclusion The levels of CRP and GDF-15 are highly expressed in children with Kawasaki disease, and the expression of ALB is low. The levels of ALB, CRP and GDF-15 are closely related to the degree of coronary artery lesions in children with Kawasaki disease. Clinically, the levels of ALB, CRP, and GDF-15 expression in children with Kawasaki disease can be detected to assess whether coronary artery lesions have occurred and the degree of injury, and timely interventional treatment can be performed to improve the prognosis of children.
