目的 探讨重睑成形术联合Stallard法 形皮瓣术治疗内眦赘皮患者的近期随访效果。方法 选择2020年1月至2021年3月医院收治的80例内眦赘皮患者为研究对象,以随机数字表法将其分成A组、B组,各40例。A组采用重睑成形术联合Stallard法 形皮瓣术治疗,B组采用重睑成形术联合Y-V成形术治疗内眦赘皮。比较两组手术相关指标、临床疗效、术后瘢痕情况、手术前后眼睑角度、内眦间距及并发症发生情况。结果 A组术后住院时间长于B组(P＜0.05),A组术中出血量高于B组(P＜0.05),A组手术时间短于B组(P＜0.05)；A组优良率(80.00%)低于B组(95.00%)(P＜0.05)； A组术后1个月、术后3个月患者与观测者疤痕评价量表(POSAS)与温哥华瘢痕量表(VSS)评分均高于B组(P＜0.05),且术后3个月,两组POSAS评分、VSS评分均低于术后1个月(P＜0.05)；术前、术后3个月,两组眼睑角度、内眦间距比较差异无统计学意义(P＞0.05),术后3个月,两组眼睑角度、内眦间距均低于术前(P＜0.05)；两组术后6个月内并发症总发生率相比差异无统计学意义(P＞0.05)。结论 重睑成形术联合Stallard法 形皮瓣术治疗内眦赘皮患者效果确切,可缩短手术时间,且安全可靠,但瘢痕更严重,存在手术创伤大、术后恢复慢等缺点。

Objective To investigate the short-term follow-up effect of double blepharoplasty combined with Stallard"s "Z" flap in the treatment of epicanthus. Methods A total of 80 epicanthus patients admitted to the hospital from January 2020 to March 2021 were selected as the research objects, and they were divided into group A and group B by random number table method, with 40 cases in each. Group A was treated with double blepharoplasty combined with Stallard"s "Z" flap, and group B was treated with double blepharoplasty combined with V-Y plasty for epicanthal. The operation-related indicators, clinical efficacy, postoperative scar, eyelid angle, medial canthal distance and complications before and after operation were compared between the two groups. Results The postoperative hospital stay in group A was longer than that in group B (P＜0.05), the intraoperative blood loss in group A was higher than that in group B (P＜0.05), and the operative time in group A was shorter than that in group B (P＜0.05). The excellent and good rate of group A (80.00%) was lower than that of group B (95.00%) (P＜0.05). The scores of Patient and Observer Scar Assessment Scale (POSAS) and Vancouver Scar Scale (VSS) in group A at 1 month and 3 months after operation were higher than those in group B (P<0.05), and 3 months after operation , POSAS score and VSS score in both groups were lower than 1 month after operation (P<0.05). Before operation and 3 months after operation, there was no significant difference in eyelid angle and medial canthal distance between the two groups (P>0.05). At 3 months after operation, the eyelid angle and medial canthal distance in the two groups were lower than those before operation (P<0.05). There were no significant difference in the total incidence of complications between the two groups within 6 months after operation (P>0.05). Conclusion Double blepharoplasty combined with Stallard"s "Z" flap in the treatment of epicanthal patients is effective, can shorten the operation time, and is safe and reliable, but the scar is more serious, and there are disadvantages such as large surgical trauma and slow postoperative recovery.
