目的 分析甲状腺乳头状癌超声参数分别与微血管密度、癌细胞恶性生物学行为之间的相关性,旨在为甲状腺乳头状癌的性质评估提供参考。方法 回顾性分析,采集我院2016年3月至2019年3月期间收治的353例甲状腺疾病患者病历资料,根据术中病理组织情况将其分为恶性组(268例)、良性组(85例),检测并对比两组超声参数[峰值强度(PI)、达峰时间(TTP)、平均渡约时间(MMT)]、微血管密度[微血管密度(MVD)]、癌细胞恶性生物学行为[增殖基因包括:BRD4、FoxM1,侵袭基因包括:Stat5、Twist1,自噬基因包括:LC3、PTEN]情况,分析甲状腺乳头状癌患者各超声参数与微血管密度、癌细胞恶性生物学行为之间的相关性。结果 与良性组相比,恶性组PI、TTP、MMT、MVD水平均较高,BRD4、FoxM1、Stat5、Twist1表达均较高,LC3、PTEN表达较低,差异均有统计学意义(P<0.05);经双变量Pearson直线相关性检验结果显示,甲状腺乳头状癌超声参数各指标(PI、TTP、MMT)与MVD呈正相关(r>0,P<0.05),与BRD4、FoxM1、Stat5、Twist1均呈正相关(r>0,P<0.05),与LC3、PTEN均呈负相关(r<0,P<0.05)。结论 甲状腺乳头状癌超声参数有其特点,且与微血管密度、癌细胞恶性生物学行为密切相关,对高度疑似甲状腺乳头状癌的患者可考虑早期进行超声检查,并对主要超声参数值进行分析,这对指导疾病恶性程度评估及治疗有一定价值。

Objective To analyze the correlation of ultrasonic parameters of papillary thyroid carcinoma with microvessel density and malignant biological behavior of cancer cell, and to provide reference for the evaluation of the nature of papillary thyroid carcinoma. Methods The retrospective analysis was performed, and the medical data of 353 patients with thyroid disease who admitted in the hospital from March 2016 to March 2019 were collected. The patients were divided into malignant women (268 cases) and benign group (85 cases) according to intraoperative pathological tissues. The ultrasonic parameters [peak intensity (PI), time to peak (TTP), mean transit time (MMT)], microvessel density (MVD), malignant biological behavior of cancer cell [proliferation genes include: BRD4, FoxM1; invasive genes include: Stat5, Twist1; autophagy genes include: LC3, PTEN] in two groups were detected and compared. The correlation of each ultrasonic parameter of papillary thyroid carcinoma patients with MVD and malignant biological behavior of cancer cell was analyzed. Results Compared with benign group, the levels of PI, TTP, MMT and MVD in malignant group were higher, the expression of BRD4, FoxM1, Stat5 and Twist1 in malignant group was higher, and the expression of LC3 and PTEN in malignant group was lower (P<0.05); Bivariate Pearson linear correlation test results showed that the ultrasonic parameters (PI, TTP, MMT) of thyroid papillary carcinoma were positively correlated with MVD (r>0, P<0.05), and positively correlated with BRD4, FoxM1, Stat5, and Twist1 (r>0, P<0.05), and negatively correlated with LC3 and PTEN (r<0, P<0.05). Conclusion Ultrasonic parameters of papillary thyroid carcinoma have its own characteristics, and are closely related to MVD and malignant biological behavior of cancer cell. For patients with highly suspected papillary thyroid carcinoma, early ultrasound examination can be considered and the main ultrasonic parameter values can be analyzed, which is of certain value to guide the evaluation of malignant degree of disease and treatment.
