目的 观察应用自制钩钢板联合重建钢板治疗髋臼后壁粉碎型骨折的临床疗效。方法 对本院2017年1月至2020年1月期间采用自制钩钢板联合重建钢板治疗的28例髋臼后壁粉碎型骨折患者资料进行回顾性分析。其中男性24例,女性4例；患者年龄为21～72岁,平均47.4岁。受伤至手术时间为2～8ｄ,平均4.2ｄ。28例患者均采用Kocher-Langenbeck入路,自制钩钢板联合重建钢板固定,将其从第1孔或中间孔处斜行45°剪断,接着将尖端折弯成70～90°,制成钩钢板,用于钩住髋臼后壁碎骨块。分析髋臼后壁骨折患者手术后的复位质量、患侧髋关节功能及相关并发症等情况。结果 28例患者术后平均获得19.5个月(7～33个月)的门诊随访。骨折并发症中一例患者合并有坐骨神经损伤。髋臼骨折复位质量应用改良的Matta评分标准进行评定：其中优为24例,良为3例,差为1例,优良率达到96.4％。髋臼骨折平均愈合时间为7.5个月(6～12个月)。采用改良的Merled"Aubigné和Postel评分系统对患侧髋关节功能进行评定：其中优为22例,良为３例,可为1例,差为２例,优良率达到89．3％。在后期的随访中,一例患者发生了髋关节创伤性关节炎,一例患者发生了患侧股骨头的缺血性坏死,随访中2例患者发生了异位骨化。结论 应用自制钩钢板对髋臼后壁粉碎性骨折进行固定,是一种可靠的内固定方式。

Objective To observe the clinical effect of self-made Hook plate combined with reconstruction plate in the treatment of Acetabular Posterior Wall comminuted fracture. Methods The clinical data of 28 cases of Acetabular Posterior Wall comminuted fracture treated with self-made Hook Plate and reconstruction plate from January 2017 to January 2020 were analyzed retrospectively. There were 24 males and 4 females aged from 21 to 72 years (mean 47.4 years) . The time from injury to operation was 2 to 8 days, with an average of 4.2 days. All the 28 patients were treated with Kocher-Langenbeck approach, self-made Hook plate combined with reconstruction plate was used to fix the fracture, which was cut 45 times from the first hole or the middle hole, and then the sharp broken end was broken into 70 ~ 90 pieces. The Hook Plate was made to catch the fragment of the posterior wall of Acetabulum. To analyze the quality of reduction, the function of the affected hip joint and the complications after operation in the patients with Acetabular Posterior Wall fracture.Results 28 patients were followed up for an average of 19.5 months (7-33 months).SOne of the complications of the fracture was associated with Sciatic nerve injury. The quality of fracture reduction was evaluated by modified Matta Score: Excellent in 24 cases, good in 3 cases, bad in 1 case, the excellent and good rate was 96.4%. The average healing time was 7.5 months (6-12 months). At the last follow-up, hip function was evaluated by modified Merled＇Aubigné and Postel scoring system: Excellent in 22 cases, good in 3 cases, fair in 1 case, poor in 2 cases, excellent and good in 89.3%. During late follow-up, one patient developed traumatic arthritis of the HIP, one patient developed ischemic necrosis of the affected femoral head, and two patients developed heterotopic ossification during follow-up.Conclusion The self-made Hook Plate is a reliable internal fixation method for the posterior wall of the Acetabulum bone fracture.
