目的:探讨MRI在颈椎氟骨症上的信号强度,评价MRI对颈椎氟骨症的诊断价值。方法:搜集2010.1月至2020.6月我院氟骨症颈椎病变患者42例和正常对照组50例,分析其MRI T1W信号强度及影像表现。结果:所有氟骨症椎体信号在T1W、T2W上不同程度减低。氟骨症椎体-肌肉T1W信号强度比值较正常对照组相同椎体序列高,统计学有显著差异(P<0.001)。42例颈椎氟骨症患者,单独后纵韧带骨化14例,单独黄韧带骨化6例,后纵韧带和黄韧带同时骨化11例;椎间盘突出32例,椎间盘变性36例,椎管狭窄30例,脊髓受压30例,脊髓变性21例。结论:氟骨症颈椎椎体MRI信号强度不同程度减低,椎体-肌肉T1W信号强度比值诊断氟中毒程度具有一定参考价值,可反应椎体内脂肪含量、分布及成骨活动的增强程度。MRI可准确评价椎体、韧带、间盘、椎管、脊髓的病理学改变,在氟骨症疾病诊断中具有重要意义。

[Abstract]
Objective:To investigate the signal intensity of MRI in cervical skeletal fluorosis and evaluate the diagnostic value of MRI in cervical skeletal fluorosis. Methods:A total of 42 patients with cervical spondylosis with skeletal fluorosis and 50 patients with normal control group were collected from January 2010 to June 2020 in our hospital. The MRI T1W signal intensity and imaging manifestations were analyzed. Results:The T1W and T2W signals of all fluorosis vertebrae were decreased in different degrees. The vertebrae - muscle T1W signal intensity ratio in skeletal fluorosis was higher than that in the normal control group with the same vertebral sequence, and the difference was significant (P < 0.001). Among the 42 patients with cervical fluorosis, 14 cases had ossification of the posterior longitudinal ligament alone, 6 cases had ossification of the posterior longitudinal ligament alone, and 11 cases had ossification of the posterior longitudinal ligament and the ligamentum flavum simultaneously. There were 32 cases of intervertebral disc herniation, 36 cases of intervertebral disc degeneration, 30 cases of spinal canal stenosis, 30 cases of spinal cord compression, 21 cases of spinal cord degeneration. Conclusion:The MRI signal intensity of cervical vertebrae in skeletal fluorosis was decreased in different degrees. The T1W signal intensity ratio of vertebral body to muscle had certain reference value in the diagnosis of fluorosis, which could reflect the content and distribution of fat in vertebral body and the enhancement of osteogenic activity. MRI could accurately evaluate the pathological changes of vertebral body, ligament, intervertebral disc, spinal canal and spinal cord, which was of great significance in the diagnosis of skeletal fluorosis.
