【摘要】目的:探讨胃肠道神经内分泌肿瘤(gastrointestinal neuroendocrine neoplasms,GI-NENs)的特点、治疗及预后,以提高诊治水平。方法:对2013年3月至2020年3月在内蒙古医科大学附属医院确诊的92例GI-NENs患者的临床资料进行回顾性分析。结果: 92例患者,男女比例:0.88:1,平均诊断年龄50.41岁;多位于直肠(80.4%),其次为胃(9.8%)、十二指肠(4.3%)、阑尾(4.3%)等;肿瘤直径:0~1cm者占76.1%,1~2cm者14.1%,大于2cm者约9.8%;浸润深度:黏膜、黏膜下层者占82.6%,肌层、外膜者17.4%;转移情况:有7.6%伴转移,92.4%无转移;治疗:共有47.8%经内镜下治疗,52.2%经外科手术治疗;1 年、3 年、5 年生存率分别是95%、94%、94%。结论:GI-NENs的发病年龄跨度较大,临床表现不典型,发病部位以直肠最为多见,病变较少浸润至肌层及以下,较少转移;不同病理分级在肿瘤的大小、浸润深度、有无转移、预后等方面的分布不同,分级越高,提示预后不佳;1 年、3 年、5 年生存率可达90%以上,肿瘤的发病部位、肿瘤分级、大小、浸润深度、转移、治疗影响患者的预后生存。

[Abstract] Objective:To investigate the characteristics, treatment and prognosis of gastrointestinal neuroendocrine neoplasms (GI-NENS), so as to improve the level of diagnosis and treatment. Methods:The clinical data of 92 patients with GI-NENS diagnosed in the Affiliated Hospital of Inner Mongolia Medical University from March 2013 to March 2020 were retrospectively analyzed. Result: 92 patients, male to female ratio: 0.88:1, average diagnosis age 50.41 years old; mostly located in rectum (80.4%), followed by stomach (9.8%), duodenum (4.3%), Appendix (4.3%); tumor diameter: 0-1cm accounted for 76.1%, 1-2cm accounted for 14.1%, > 2cm about 9.8%; depth of invasion: mucosa, submucosa accounted for 82.6%, muscularis, adventitia accounted for 17.4%; metastasis: 7.6% with metastasis, 96.4% without metastasis; treatment: a total of 47.8% were treated by endoscopy, 52.2% by surgery; 1-year, 3-year, 5-year survival rates were 95%, 94%, 94% respectively. Conclusion:the age span of GI-NENS is large, and the clinical manifestations are not typical. The most common site of GI-NENS is rectum. The lesions are less invasive to the muscular layer and less metastasis. The distribution of different pathological grades in tumor size, depth of invasion, metastasis and prognosis are different. The higher the grade is, the worse the prognosis is ,The location, grade, size, depth of invasion, metastasis and treatment of the tumor affect the prognosis of the patients.
