【摘要】目的:探究混合抗生素骨水泥填充技术对糖尿病足患者创面愈合情况、足背动脉血流动力学及下肢功能的影响。方法:以2018年9月-2020年12月本院接治的88例糖尿病足患者作为研究对象,将其随机分为实验组和对照组,每组患者各44例。其中对照组给予常规清创处理,实验组在对照组基础上混合抗生素骨水泥填充技术治疗。观察2组患者临床疗效、创面愈合情况、足背动脉血流动力学及下肢功能情况。结果:研究组总有效率为88.64%,高于对照组61.36%(P<0.05);治疗前,2组患者创面面积大小、经皮氧分压和创面pH差异都无统计学意义(P>0.05);治疗后,2组患者的创面面积大小和创面pH都比治疗前有所减小,经皮氧分压有所升高(P<0.05);且研究组创面面积大小显著小于对照组,经皮氧分压明显高于对照组,创面pH明显小于对照组(P<0.05);治疗前,2组糖尿病足患者足背动脉血管内径、血流量和峰值流速差异都无统计学意义(P>0.05);治疗后,2组糖尿病足患者的足背动脉血管内径、血流量都比治疗前有所增加,峰值流速有所降低(P<0.05);且研究组血管内径和血流量均明显大于对照组,峰值流速小于对照组(P<0.05);另外,2组患者治疗前腓总神经感觉传导速度和胫神经运动传导速度差异无统计学意义(P>0.05);相比治疗前,治疗后2组患者腓总神经感觉传导速度和胫神经运动传导速度都有所上升(P<0.05);且治疗后,研究组腓总神经感觉传导速度优于对照组,胫神经运动传导速度也优于对照组(P<0.05)。结论:混合抗生素骨水泥填充技术能够有效缓解糖尿病足患者的症状,快速改善创面愈合情况,纠正足背动脉血流动力学异常,提高下肢功能,两药相互辅助,具有协同作用,能显著提高临床疗效。

[Abstract] Objective: To explore the effects of Bone cement filling with mixed antibioticson wound healing, dorsal pedal artery hemodynamics and lower limb function in patients with diabetic foot. Methods: 88 patients with diabetic foot treated in our hospital from June 2017 to December 2018 were randomly divided into two groups, 44 in the study group and 44 in the control group. The control group was treated with Moist Burn Ointment and the research group was treated with Fuyuan Huoxue Decoction on the basis of the control group. The clinical efficacy, wound healing, dorsal pedal artery hemodynamics and lower limb function were observed in both groups. Results: The total effective rate of the study group was 88.64%, which was higher than 61.36% of the control group (P < 0.05). Before treatment, there were no significant differences in wound area, percutaneous oxygen partial pressure and wound pH between the two groups (P > 0.05). After treatment, the wound area and wound pH in both groups decreased, and the percutaneous oxygen partial pressure increased (P < 0.05). Moreover, the wound area of the study group was significantly smaller than that of the control group, and the percutaneous oxygen partial pressure was significantly higher than that of the control group, and the wound pH was significantly lower than that of the control group (P < 0.05). Before treatment, there was no significant difference in the intravascular diameter, blood flow and peak velocity of dorsal pedal artery between the two groups (P > 0.05). After treatment, the intravascular diameter and blood flow of dorsal pedal artery increased and the peak velocity decreased in both groups (P < 0.05). Moreover, the intravascular diameter and blood flow in the study group were significantly larger than those in the control group, and the peak flow velocity was less than that in the control group (P < 0.05). In addition, there was no significant difference in sensory conduction velocity of common peroneal nerve and motor conduction velocity of tibial nerve between the two groups before treatment (P > 0.05). Compared with before treatment, sensory conduction velocity of common peroneal nerve and motor conduction velocity of tibial nerve increased in both groups after treatment (P < 0.05). After treatment, the sensory conduction velocity of common peroneal nerve and motor conduction velocity of tibial nerve in the study group were better than those in the control group (P < 0.05). Conclusions: Bone cement filling with mixed antibioticscan effectively alleviate the symptoms of diabetic foot patients, rapidly improve wound healing, correct the abnormal hemodynamics of dorsal pedal artery, improve the function of lower limbs. The two drugs complement each other and have synergistic effect, which can significantly improve the clinical efficacy.
