目的 本文旨在回顾性的分析非移植患者他克莫司(Tacrolimus,TAC)血药谷浓度与血常规指标之间的相关性,为TAC的临床使用提供依据。方法 选择2020年4月至2022年4月在内蒙古医科大学第一附属医院进行TAC血药谷浓度监测的51例非移植患者为研究对象,记录TAC使用剂量和血药谷浓度监测结果,通过皮尔森相关分析(Pearson)研究血常规指标与TAC血药谷浓度的关系。结果 51例患者包含系统性红斑狼疮患者21例(41.18%),肾病综合征患者14例(27.45%),抗合成酶综合征患者6例(11.76%),类风湿患者和系统性硬化症患者各2例(3.92%)。使用患者体重和服药剂量对TAC血药谷浓度进行校正后,皮尔森相关分析结果显示,中性粒细胞计数和血小板计数与校正后的TAC血药谷浓度显著正相关(相关系数:0.287和0.281,P<0.05),而血小板平均体积与校正后的TAC血药谷浓度显著负相关(相关系数:-0.344,P<0.05)。结论 非移植患者血常规指标与TAC血药谷浓度有一定相关性,在指导TAC使用时要综合考量TAC暴露剂量不足或过量所导致的不良反应。

Objective To retrospectively evaluate correlation between correlation between trough plasma concentration of tacrolimus (TAC) and hematologic parameters in non-transplanted patients. Methods A total of 51 non-transplanted patients who underwent plasma TAC concentration monitoring in the First Affiliated Hospital of Inner Mongolia Medical University were selected from April 2020 to April 2022. TAC dosage and trough plasma concentration were recorded.The correlation between trough plasma concentration of TAC and hematologic parameters was analyzed by Pearson correlation analysis. Results The clinical diagnosis of 51 non-transplanted patients consisted of 21 patients (41.18%) with systemic lupus erythematosus, 14 patients (27.45%) with nephrotic syndrome, 6 (11.76%) patients with anti-synthetase syndrome, 2 patients (3.92%) with rheumatoid disease and 2 patients (3.92%) with systemic sclerosis. After plasma trough concentration of TAC was adjusted by TAC dosage and body weight of patients, Pearson correlation analysis illustrated that neutrophil and platelet counts was positively correlated with dosage-adjusted plasma trough concentration of TAC (Pearson's correlation:0.287 and 0.281, respectively, P<0.05), besides, a significantly negative correlation was found between dosage-adjusted plasma trough concentration of TAC and mean platelet volume (Pearson's correlation:-0.344, respectively, P<0.05). Conclusions In our study, a correlation between plasma trough concentration of TAC and hematologic parameters was found in non-transplanted patients, doctors and clinical pharmacists need comprehensive consideration of TAC use to avoid adverse reactions due to TAC exposure with inappropriate dose.
