[摘要]
目的:本研究通过比较 “传统自由手技术”同“实时动态导航技术”两种技术在“双侧颧骨种植术”中的手术时长、手术安全性(上颌窦及眶底眶外侧壁损伤)、手术精准性三方面的效果,从而进一步评价实时动态导航技术应用于患者穿颧种植的效果。为实时动态导航技术应用于颧骨种植提供相关的理论基础及实验依据,以期手术的广泛普及。方法:通过采集符合纳入条件的患者的影像学数据,首先进行术前双颧种植方案设计,并运用3D打印技术制作仿真植入模型。共6个模型,随机分为2组(A组实验组,B组对照组),每组3个植入模型,实验组为“实时动态导航技术”组,对照组为“传统自由手技术组”。再将植入模型固定于口腔教学头模,并进行配准钉植入、导航定位器植入安放及导航配准等相关术前准备工作,进而模拟真实患者进行“双侧颧骨种植术”。比较二者的手术时长、手术安全性与精准性。结果:1.实验组与对照组在“双侧颧骨种植术”中的手术时长具有统计学差异(P<0.05);2.本实验中实验组比对照组在“双侧颧骨种植术”中的手术安全性(上颌窦及眶底眶外侧壁损伤)更高;3.实验组与对照组在“双侧颧骨种植术”中的手术精准性无统计学差异(P>0.05)。结论:实时动态导航技术可以有效降低手术操作时长,提高手术效率,且安全性更高。总之,实时动态导航技术辅助双侧颧骨种植的初步结果是值得肯定的。

[Abstract]
Objective This study compared the effects of "traditional free hand technology" and "real-time dynamic navigation technology" in three aspects: operation duration, operation safety (maxillary sinus and lateral orbital wall injury of orbital floor) and operation accuracy in "bilateral zygomatic implantation", so as to further evaluate the effect of real-time dynamic navigation technology in patients" zygomatic implantation. To provide the relevant theoretical and experimental basis for the application of "real-time dynamic navigation technology" in zygomatic implant, in order to popularize the surgery. Methods By collecting the imaging data of patients who met the inclusion conditions, the preoperative double zygomatic implant scheme was designed, and the 3D printing technology was used to make the simulated implant model. A total of 6 models were randomly divided into 2 groups (experimental group A and control group B), with 3 implanted models in each group. The experimental group was "real-time dynamic navigation technology" group and the control group was "traditional free hand Technology Group". Then the implant model was fixed on the oral teaching headform, and the pre-operative preparations such as registration nail implantation, navigation locator implantation and navigation registration were carried out, so as to simulate the real patient for "bilateral zygomatic implantation". The operation duration, safety and accuracy were compared. Result 1.There was significant difference in the operation duration between the experimental group and the control group (P<0.05). 2. In this experiment, the safety of the experimental group in the "bilateral zygomatic implantation" was higher than that of the control group (injury of maxillary sinus and lateral orbital wall of orbital floor); 3. There was no significant difference in the surgical accuracy between the experimental group and the control group(P>0.05).Conclusions Real-time dynamic navigation technology can effectively reduce the operation time, improve the operation efficiency, and have higher safety. In conclusion, the preliminary results of bilateral zygomatic implantation assisted by real-time dynamic navigation technology are positive.
