[摘要]
目的 研究睫状肌麻痹散瞳验光对4~14岁儿童青少年屈光状态的变化。方法 将2023年1月到2023年7月就诊于内蒙古医科大学附属医院近视眼激光中心的屈光不正患者,经纳入排除标准,共纳入218眼,根据年龄分为4~8岁组(70眼),9~14岁组(148眼)。分析不同年龄组间睫状肌麻痹散瞳(以下简称散瞳)验光后屈光状态的改变。 结果 在不同年龄组间散瞳均能改变受试者的屈光度。4~8岁组散瞳前等效球镜、球镜分别较散瞳后降低,且差异具有统计学意义(P<0.05),散瞳后柱镜值升高,差异具有统计学意义(P<0.05)。9~14岁组散瞳后等效球镜、柱镜值均有所升高,差异具有统计学意义(P<0.05);散瞳前后组比较,球镜没有统计学意义(P>0.05)。 结论 散瞳验光能为屈光不正儿童提供有效的验配方案。4~8岁儿童采用阿托品散瞳能明显改变屈光度,能降低球镜、柱镜、和等效球镜的绝对值,更趋近于正视状态,9~14岁青少年采用美多丽散瞳可以降低等效球镜与散光度值,改善儿童青少年的视觉质量。

Objective To investigate the effect of cycloplegic mydriatic refraction on refractive status in children and adolescents aged 4-14 years. Methods A total of 218 eyes of patients with refractive errors in the Myopia Laser Center of the Affiliated Hospital of Inner Mongolia Medical University from January 2023 to July 2023 were included according to the inclusion and exclusion criteria. According to the age, they were divided into the 4-8 years old group (70 eyes) and the 9-14 years old group (148 eyes). To analyze the changes in refractive status after cycloplegic mydriatic refraction in different age groups. Results Mydriasis changed the refraction of subjects in different age groups. In the 4-8 years group, the spherical equivalent and spherical equivalent before mydriasis were lower than those after mydriasis, and the difference was statistically significant (P < 0.05), and the cylinder value increased after mydriasis, and the difference was statistically significant (P < 0.05). In the 9-14 years group, the spherical equivalent and cylinder values increased after mydriasis, and the difference was statistically significant (P < 0.05). There was no significant difference in spherical refraction before and after mydriasis (P > 0.05). Conclusion Mydriatic optometry can provide an effective fitting scheme for children with ametropia. The use of atropine mydriasis in children aged 4-8 years can significantly change the diopter, reduce the absolute value of spherical, cylindrical and spherical equivalent, more close to the emmetropia state. The use of mydriasis in children aged 9-14 years can reduce the spherical equivalent and astigmatism, and improve the visual quality of children and adolescents.
