目的:本文旨在分析精神疾病患者中药物代谢酶基因CYP3A4、CYP1A2、CYP2C19、CYP2D6的分型情况,并探讨其在双相障碍,精神分裂症,抑郁症患者中的分布。方法 收集2020年1月—2023年10月在我院情感障碍科住院的难治型精神疾病患者60名。入组患者经精神疾病诊断标准ICD-10诊断为抑郁症,双相情感障碍,精神分裂症;采用核酸飞行质谱基因分析系统进行药物代谢酶检测。分析药物代谢酶CYP3A4,CYP1A2,CYP2C19,CYP2D6在精神疾病患者中的分布。结果:CYP3A4的快代谢型(Extensive Metabolizer,EM)占比96.7%,CYP1A2超速型代谢(Ultrarapid Metabolizers,UM)和EM代谢型分布为48.33%和51.67%,CYP2C19和CYP2D6在精神疾病中弱代谢(PM,Poor Metabolizer)患者较多。需要及时关注患者的CYP450酶分型。结论:药物代谢酶在三种精神疾病中的分布情况可提示医生合理选择药物,对个体化用药的实施具有重要意义。

Objective: To study the genotype distribution of cytochrome CYP3A4, CYP1A2, CYP2C19, CYP2D6 in psychiatric disorders and the genotype frequency are compared bipolar disorder, schizophrenia, and depression. Methods: Sixty patients collected in Emotional Disturbance Sector from January 2020 to October 2023. All enrolled patients were refined ICD-10 diagnostic criteria for depression, bipolar disorder, and schizophrenia. Cytochrome P450 enzyme were assayed using a nucleic acid flight mass spectrometry gene analysis system. Categorical statistics on the distribution of four metabolic enzymes of the cytochrome P450 enzyme family, CYP3A4, CYP1A2, CYP2C19, and CYP2D6, in three types of psychiatric patients. Results: Extensive Metabolizer (EM) of CYP3A4 accounted for 96.7%, Ultrarapid Metabolizers (UM) and EM metabolizers of CYP1A2 were distributed in 48.33% and 51.67%, and CYP2C19 and CYP2D6 were more frequent in psychiatric disorders in Weak Metabolizer ( PM, Poor Metabolizer) patients were more frequent. Patients' CYP450 enzyme typing needs to be followed in time. Conclusion: The distribution of CYP450 enzyme in three psychiatric disorders may prompt physicians to rationally select medications, which is important for the implementation of individualised medication.
